“In the ACO model, an organization voluntarily takes on the responsibility of caring for a defined population of patients and agrees to meet cost and quality benchmarks in exchange for part of the ‘savings’ derived from delivering better quality of care” (Highfill & Ozcan, 2016). I decided to research if the organization where I am employed was using one of the innovative models of care and they are. I currently work for Hialeah Hospital and they are using The Bundled Payments for Care Improvement Advanced (BPCI Advanced) Model. “The Model aims to support healthcare providers who invest in practice innovation and care redesign to better coordinate care and reduce expenditures, while improving the quality of care for Medicare beneficiaries. BPCI Advanced qualifies as an Advanced Alternative Payment Model (APM) under the Quality Payment Program” (CMS.gov, 2021). The goals of the BPCI Advanced Model are to structure the way care is provided, to create a commitment between health care providers, caregivers, and patients, and to hold organizations financially accountable through review of data and patient/caregiver feedback. Hialeah Hospital does this by reviewing Performance Improvement data, such as hospital acquired pressure ulcers, complications of joint replacement surgeries, effectiveness of new service lines implemented, and patient satisfaction surveys, and so many more. As a Nurse Manager, I have been involved in reviewing and collecting data to help improve patient outcomes. I work closely with the nurses and other disciplines to ensure we are doing the absolute best we can to ensure patient safety and satisfaction.

This model affects interprofessional collaboration because it requires physicians, nurses, physical therapists, wound care, dietitians, and so many other interdisciplinary specialties to work together to improve patient satisfaction and outcomes. Since each interdisciplinary team member is responsible for some portion of the outcome, each is motivated to provide the best care in order to maximize reimbursement.

In an article and study by Gordon and colleagues, they had determined the use of an innovative care model for telemonitoring for health care management for patients with complex chronic needs (CCN). In this interesting article, they discussed finding evidence-based holistic strategies, rather than a single-condition approach in order to promote and manage chronic conditions. Nearly 40% of American have two to three chronic conditions and about 30% have four or more (Gordon et al., 2020). Comorbidities are associated with a higher risk of hospitalizations, higher health care costs, higher morbidity risk and a higher risk of death. “Current practice models are overburdened and often under-resourced to comprehensively and holistically address, monitor, and stabilize the complex health care needs of this high-cost, high-risk population” (Gordon et al., 2020). According to this article, several systematic reviews have concluded the use of telemonitoring can lead to improved clinical outcomes and reduce the costs for some chronic conditions. Some chronic conditions listed include depression, anxiety, diabetes, hypertension and COPD. “Because of inadequate health care delivery, patients are seeking opportunities outside of traditional care delivery models to seek better experiences within the health care system (Gordon et al., 2020).

Collaboration and coordination depends on the clinician (usually an NP) who is designated as the central coordinator of the patients’ needs but as well as the needs of the interdisciplinary team. I believe it can decreased the workload of health care providers, staying in an office building verses traveling from hospital to hospital and back to an office building. It is suggested that telemonitoring could be embedded in a novel integrated model of care, especially for patients with multiple co-morbidities. During the pandemic, the use of the telemonitoring was increased, to reduce the spread of the virus, to protect the patients, and to protect the health care professionals. This innovative model of utilizing telemonitoring in patient care is beneficial in many ways: reducing costs, reducing or avoiding hospitalizations, and it can be beneficial for individuals that live in rural areas.

Interprofessional Teamwork Innovation Model is to promote communication and patient-care centered coordination of care. The goal is to improve the communication, patient care to prevent readmissions, same day ed visits. This uses for the quality improvement system and focuses on the Plan-Do-Study-Act framework in which every detail will be assessed and to have an action plan. This also helps in providing good communication between the health care team and the patient and family.
